ESSENTIAL DUTIES:
Under limited supervision from the Social Service Supervisor or Human Services Supervisor. This long-term care/disability unit position will focus on assessment for community services to individuals, families, and the community. This full-time, benefit-eligible position is located at the Carlton County Public Health & Human Services building with work being performed primarily in client’s homes and in the community. Upon completion of a probationary period, a hybrid schedule between office building, community, and remote work may be approved. In addition, this position will include professional public health nurse duties or social worker duties to ensure the proper administration and coordination of established public health programs per state statute, regulations, and guidance. Collaborate closely with local, regional, state, and Tribal health systems, governmental systems, and other agencies to develop interdependent perform professional registered nurse/PHN or social work duties to ensure the proper administration and coordination of established long term care/public health and disability unit programs. Must be skillful at developing and working with multi-disciplinary teams, families, and general public. Exercise independent judgment, exercise strong oral and written communication skills. Some evening and/or weekend work may be required.
EDUCATION/EXPERIENCE:
PHN:
Must have RN Licensure from the State of MN Board of Nursing and Minnesota Public Health Nursing certificate; Bachelor’s degree in Nursing from a university or college accredited by the National League of Nursing (PHN)
SW:
Bachelor’s degree in from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field OR A bachelor’s degree from an accredited four year-college or university with a major in any field and Three (3) years of public health and human service experience with two (2) years in the program area. Or an equivalent combination of education and experience sufficient to perform the essential functions of the job.
RN:
Must have RN Licensure from the State of MN Board of Nursing and minimum associates’ degree in nursing.
