The US Labor Department has given a citation and proposed a penalty of $308 , 000 to a plumbing contractor in Macon after a worker died due to safety violations . Pyles Plumbing and Utility Contractors Inc . failed to ensure the safety of a 20 - year - old worker , who lost his life in a trench collapse . Another worker was also injured and hospitalized . The federal investigators from the Occupational Safety and Health Administration found that the incident could have been prevented if the contractor had followed the required safety procedures . The violations include not providing proper trench shields and a safe means of exiting a trench box . The employer also allowed employees to work in a trench without adequate protection and failed to provide proper training to avoid hazards . OSHA has proposed the penalties , emphasizing the importance of following safety protocols in the construction industry . Pyles Plumbing and Utility Contractors Inc . has the option to comply , request an informal conference , or contest the findings within 15 business days . OSHA has recently announced enhanced enforcement to prevent injuries related to trenching and excavation collapses .
