An investigation by the US Department of Labor has found that HBD/Thermoid once again exposed its employees to unsafe conditions . As a result , they are facing an additional $321K in penalties . This is the eighth time that OSHA has investigated the Bellefontaine plant since 2013 . The violations include inadequate machine guarding , unsafe electrical practices , lack of training , and failure to implement lockout/tagout procedures . OSHA has criticized the company for not prioritizing workplace safety and putting their employees at risk . The investigation was opened after a 25 - year - old employee suffered severe injuries at the same facility in June 2023 . In December 2023 , OSHA issued more violations and proposed penalties totaling $389 , 534 . HBD/Thermoid Inc . is a subsidiary of HBD Industries and operates manufacturing facilities in Bellefontaine , Chanute , and Oneida . The company produces industrial hoses , conveyor belting , and duct materials for various industries .
