   BASIC FUNCTIONS: 
   This position is responsible for providing social worker and case coordination services to the clients of the San Juan County Health & Community Services (HCS) Department, focusing on pregnant and postpartum people and their families enrolled in Growing Families and WIC, and on seniors receiving services through the County’s senior services programs. This position will also assist in coordinating and convening the San Juans Reproductive Health Coalition, working with community partners across the county to improve access to coordinated reproductive health services for all persons residing in San Juan County. Additionally, this position will serve as an outreach coordinator for the San Juan County Latinx community, working closely with the HCS Communications Coordinator; as such, it is highly preferred that the individual be bilingual English/Spanish.
   MINIMUM QUALIFICATIONS:
   .
   Minimum of bachelor’s degree in social work or behavioral sciences AND two (2) years of supervised experience indirect counseling services AND one year of experience in community social services or public health social services.; OR any equivalent combination of education and experience that demonstrates the ability to perform the essential functions of the job. 
   Registered Counselor with Washington State.
   Valid Washington State Driver’s license. 
   Pass background check per Department criteria. 
   Experience working within reproductive health programming and/or coalition building and coordination.
   PREFERRED QUALIFICATIONS:
   Strongly prefer master’s degree from an accredited school in psychology, social work, marriage and family therapy, or educational psychology with an internship or practicum experience in direct counseling services AND one year of supervised experience in direct counseling services to clients or comparable Washington State certification. 
   Highly prefer bilingual English/Spanish.
   EXAMPLES OF ESSENTIAL DUTIES: 
   Social Work & Care Coordination: 
   Provide brief, culturally relevant and appropriate counseling, crisis intervention, and care coordination services for pregnant people and their families under the Growing Families and WIC programs within the Community Health Services Division; also provides services for senior clients receiving services under the SJC HCS senior programs. Establishes and maintains on-going therapeutic relationships with clients, as indicated and needed per program guidelines. Applies appropriate developmental knowledge to individual family and client situations, as well as in group settings. Applies knowledge of specific populations, including but not limited to pregnant people, infants, young children substance users, domestic violence, homelessness, disabilities, and other high-risk factors. Coordinates community resource options with clients, including but not limited to DSHS, CPS, community mental health and drug and alcohol treatment centers, employment support services, educational resources, and other educational, health, and social service providers and programs. Participates on multidisciplinary teams in health, educational, and other settings for case planning and evaluations. Determines when a referral for more in-depth services is necessary, makes the referral, and applies appropriate follow-through.
   Coalition Building & Implementation: 
   Participates in community resource development, grant writing, systems planning, and interagency coordination activities. Develops and maintains good working relationships with other health, counseling, and community professionals (including therapists, doctors, community service officers, law enforcement, and schools). Assists in convening a regular meeting of the San Juans Reproductive Health Coalition, working productively with community partners to coordinate and improve access to high-quality, evidence-based reproductive and sexual health services for all individuals living in San Juan County.
   Communication & Outreach:
   Serve as a culturally appropriate conduit for outreach to and communication with the San Juan County Latinx community. Work within the HCS Department as an advocate, ensuring the Latinx voice and community is included in program planning and development. Work closely with the HCS Communications Coordinator on outreach to the Latinx community.
   Cultural Competency:
   Apply determinants of health to develop culturally responsive interventions with specific populations; and use epidemiological data/concepts to analyze the determinants of health when developing service plans and approaches.
   Direct Service Provision:
   Provide direct social work and care coordination services to San Juan County residents in assigned program areas; maintain confidential client records in the HCS Department’s Electronic Medical Record system, Nightingale Notes, (including intake assessment, problem identification, case planning, implementation, progress notes, and outcome evaluation) in coordination with public health nurses; coordinates special agency projects as assigned; and act within the scope of responsibilities, working as a public employee with courtesy and professionalism, and adhering to the highest standard of ethics in accordance with RCW 42.52.

   PHYSICAL AND MENTAL REQUIREMENTS
   :
   Work requires continuous sitting and operation of a computer including keyboard and mouse operation.
   Frequent standing and walking.
   Involves talking and hearing in person and on the phone. 
   Vision abilities required by this job include close vision and the ability to differentiate colors and read text and information on computer monitors.
   Deals with customer stress and deadlines, which may require overtime, flexibility of time.
   This position requires attention to detail and strong communication skills.
   WORKING CONDITIONS:
   The Public Health Social Worker may work in a variety of settings, including home, office, schools, businesses, and/or other community settings. Physical settings may be unsanitary and potentially dangerous. This position may encounter occasional traumatic results that require post-event counseling. Position may work occasional weekends and may need to respond to off-hour calls. It is anticipated that travel will be less than 20% of the time. There may be possible exposure to blood borne pathogens and other communicable diseases. There is potential for exposure to hostile individuals and settings. This position requires the ability to maintain maximum flexibility regarding schedules and workloads.
