The U . S . Chemical Safety and Hazard Investigation Board ( CSB ) released a report today on the fatal acid release that happened in LyondellBasell La Porte Complex in Texas last year . The report found out that two contract workers died and two others were seriously injured after the inadvertent removal of pressure retaining components of a valve caused the release of 164 , 000 pounds of an acetic acid mixture . The facility ' s property damage was estimated to be $40 million . The CSB recommends improving the design of these valves and taking other protective actions like signage and training to prevent similar incidents from happening in the future . The report also discusses two key safety issues . So basically , there were some incidents that happened with these plug valves and they need to be re - designed so that it ' s harder to remove certain parts . The people working on it weren ' t given enough training or procedures , which led to a tragic accident . The CSB is recommending changes to prevent this kind of thing from happening again .
