Area Agency on Aging (AAA) Vaccine Care Specialist
(AACOG)
This is a responsible, non-supervisory position requiring familiarity with elderly, various rules & regulations regarding Aging program and services; Coordinates referrals for service within Area Agency on Aging (AAA) to ensure prescribed needs of client are carried out in accordance with (Older Americans Act) OAA/service definitions; Provides technical assistance, development and implementation of programs and services for the elderly in the 13 counties of the Area Agency on Aging.
Prepares and processes client intakes for submission into specialized database or equivalent; Conducts over the phone assessments to determine client’s needs, and provides resource information or referrals to elderly adults and/or their family members and documents intake of client information in physical or electronic records; Ensures necessary case and services follow-up is conducted and ongoing monitoring of clients status; Maintains current, complete, and accurate required data in agency database; Maintains follow-up with clients to promote communication and enhance client relationship with agency staff; Collects, analyzes and prepares weekly, monthly, and yearly reports for Vaccine Coordinator, AAA Manager, Health and Human Service Commission (HHSC) and other reports as needed; Assists with other projects, special events, and referrals to Older Adults in the 13-county region. Maintains confidentiality when handling client records; Assists Vaccine Outreach Coordinator in compiling client service efforts and outcomes; Adheres to quality assurance plan, policies and procedures per internal and external agencies; Properly documents time in ADP or its equivalent. Performs other job related duties as assigned.
Desired Education/Job Experience:
Associates degree in Social Services, Public Administration or related field; or any equivalent combination of experience and training which provides the knowledge, skill, and abilities to perform this role; Spanish is preferred, but not required.
Other Qualifications:
Strong attention to detail and computer proficiency; excellent verbal, written, and interpersonal communication skills; Ability to work independently with minimum supervision while meeting program deadlines; Demonstrate good judgement and discretion; Ability to represent AACOG-AAA in a courteous professional manner.
This position is a Pay Grade 8, with a starting hourly rate of $16.52. This is an non-exempt position. Open until Filled.
