Williamson County Health Department is accepting applications for a full-time Care Coordinator. This position provides case management services for families of children – birth to 21 years of age – with physical disabilities who meet financial and diagnostic guidelines; completes home visits and assessments for infants and children who may be at risk for medical or developmental problems; conducts needs assessments for pregnant and post-partum women and connects them to community resources; and conducts community outreach activities. This position requires medical charting.
A Bachelor’s degree from an accredited college or university in social services, education or other related field is preferred.
Salary for those fully meeting the minimum qualifications is $20.00 per/hour.
