   Position will be located at the North Fork Annex in Hotchkiss. 
   Delta County is accepting applications for the position of Case Manager - Options for Long-Term Care.  This position performs journey level case management providing a full range of intake and/or ongoing case management services for all programs offered under Single Entry Point (SEP) and Colorado Options for Long Term Care (OLTC).
   ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
   Assesses clients for physical and mental disabilities, level of functioning and support systems, makes determination as to program appropriateness.
   Assesses client’s medical needs. Counsels use of medical and/or psychiatric treatment. Formulates plan to maintain client in healthful environment.
   Develops a case plan utilizing informal support systems, community resources, and development of new resources for specific needs of client. Develops, locates, and coordinates services in the community to maintain the client’s highest level of independence with cost containment criteria.
   Determines the need for placement of clients in long term facilities such as adult foster care, alternative care facility, and nursing home facility. Completes the proper assessment document and facilitates admissions.
   Requirements:  
   Bachelor’s degree in human services, social service, psychology or related field required
   Two years of experience in case management preferred
   Salary Range:  
   $38,847 - $47,568/year
