The City of Williamsburg Human Services Department is hiring a full-time Family Services Case Manager to work as part of our team addressing the needs of City youth and their families through the provision of community-based wrap-around services. The Family Services Case Manager will serve to address the health needs of City children and families. Services are delivered in cooperation with family members to enhance the family’s ability to find and utilize services and have better health outcomes. The Family Services Case Manager works as a team member with the Care Team Case Manager and Registered Nurse to coordinate services for both children and their families. In addition to working with families enrolled in the Care Team program, the Family Services Case Manager will assist the Youth and Family Services Coordinator in providing assessment, case management, and support services to other City families and youth.
Position is full-time, subject to available funding, and requires a flexible schedule to meet the needs of children and their families.
Qualifications:
Education and Experience:
Minimum of Bachelor’s Degree in Social Work or related field;
Minimum of 2 years’ experience and/or training within public and/or private family service programs
Strong verbal, written, and oral communication skills
Special Qualifications:
Valid Virginia Driver’s License or obtain within 90 days
Acceptable Criminal history, Child protective services and DMV background checks
