   This is a full time exempt position
   Mahoning County Juvenile Court serves as the Administrative Agent of MCFCFC and as such hires the position. Normal Working Hours: 40-hour week, flexible and variable. Normally M-F, 8 hour shift
   General Job Summary:
   This position will be working in collaboration with Board of DD, Children Services Board, Mental Health and Recovery Board, and Juvenile Court. Responsible for implementing the county service coordination process for eligible families in Mahoning County, facilitating communications to those involved in providing services to individuals and families, and monitoring and reporting outcomes.
   Minimum Qualifications:
   The following qualifications are considered as guidelines:
   Bachelor degree required. Certification / licensure preferred in Social Work or similar field
   Experience and Proficiency with computer applications, including Microsoft Office
   Demonstrated ability to coordinate, facilitate, and reach consensus among divergent groups.
   Extensive knowledge of services available in Mahoning County.
   Good organizational and time-management skills
   Willing to work flexible hours, including evenings
   Ability to travel regularly within county and occasionally outside of county.
   Alternatives to the above as approved by the Appointing Authority.
   Essential Functions:
   Develop collaborative partnerships between agencies for services provision.
   Provide outreach to referral sources and assistance in the completion of referral materials.
   Accept referrals and determine level of service entry.
   Develop and maintain case files and all paperwork and assure confidentiality of records.
   Responsible for appropriate filing, pulling and refiling of case files as needed.
   Coordinate and facilitate family team meetings.
   Provide technical assistance and coaching to teams / agencies.
   Assure that the WrapAround process is utilized.
   Prepare and provide written documentation and distribution of Family Service Plan for each student in county service coordination mechanism.
   Research, identify, and organize supports needed by families.
   Recruit family advocates.
   Assure that screening tools and outcome measures are completed within timelines.
   Report progress and outcomes to team, funding source, and MCFCFC.
   Complete documentation for case closures and assist with transition to other services.
   Assist with development of data collection system.
   Complete data entry and maintain database of youth/families in service coordination.
   Compile data required by the state or MCFCFC and report as appropriate.
   Coordinate and monitor utilization of multiple funding sources.
   Develop, administer, and maintain shared cost allocation database.
   Prepare reports, agenda, notifications, cases to be reviewed, and funding requests for the Clinical and Funders Committees.
   Participate in on-going review of the county Service Coordination Mechanism.
   Serve as Lead for all accompanying processes including data systems.
   Reduce number of days youth are in residential care by working with team and provider to develop expected outcomes, timelines for achievement, monitoring for appropriateness, quality, cost effectiveness, and assuring supports are in place to return youth to family and community.
   Other duties as assigned.
   Unusual Working Conditions:
   Must be able to stand for four hours or more. Frequently requires lifting and carrying equipment / materials 1 – 20 lbs. Occasionally requires lifting and carrying equipment / materials 21 – 40 lbs.
   May be exposed to communicable diseases, unpredictable or challenging behaviors, and chemicals commonly found in an office environment (i.e., toner, correction fluid).
   The final applicant selected for this position will be required to submit to drug test, Wonscore , and criminal background check.
