The Health Center seeks candidates for the position of Community Development Specialist to perform activities within the Health Planning & Policy Team.
The Community Development Specialist is responsible for collaborating with community partners and member, facilitating the assessment of community needs and implementing evidenced-based strategies to address those identified needs. The Specialist is also responsible for securing and leveraging community resources that enable assigned community health improvement initiatives to be successful.
Position Status
: Full-time employee. This position is eligible for a partial remote work schedule following a six-month training period.
Duties include:
Assists or takes a lead role (as assigned) in developing plans, implementing specific community health improvement programs for civic groups, businesses and the community?
Develops and maintains cooperation between Local Public Health System (LPHS) partners to maintain and update Community Health Assessments and Community Health Improvement Plans?
Promotes community wellness and health?in all policies discussions with schools, industry, and other LPHS partners?
Sets priorities for planning activities related to community partnerships to ensure a comprehensive approach to community-based health improvement plans and programs?
Attends and actively participates on community planning teams as needed?
Maintains considerable knowledge of public health and coalition building?
Works effectively with local, state and federal government officials, public health staff, health and social service agency personnel and community groups?
Works independently with minimum supervision?
Develops and manages grant and contract proposals as assigned?
Communicates regularly with the CCPHC management team and other stakeholders to ensure expedient and effective implementation of any initiative?
Collaborates and/or facilitates community health needs assessments, implementing evidenced-based strategies to address identified needs, securing and leveraging resources, and enabling identified community health improvement initiatives to be successful within their assigned area?
Maintains accurate, organized work records, documents and files?
Proactively provides feedback for improvements that improve quality, efficiency, cost savings,?productivity, systems, and/or procedures?
Maintains strict confidentiality; adheres to all HIPAA guidelines/regulations??
Reports for special assigned duties during a public health emergency, if applicable?
Other duties as assigned
Position Qualifications:
A?Bachelor’s Degree from an accredited four-year college or university in?health planning, health education, community development, community planning or related field?is required; an equivalent combination of education and/or related experience may be considered in lieu of a?Bachelor’s?degree. A Master’s Degree in?public health?or related field is preferred.?
Excellent communication and computer skills?are?required?
Two or more years related experience?is?preferred?
Grant writing and grant management experience?and/or?good working knowledge of community health assessments and health impact assessments?is?preferred?
A current driver’s license and auto insurance are required?
